So you need an MPTCP-aware endpoint at the other end of the connection? I was imagining something that could be implemented unilaterally by a savvy home/office user just to ease the disruptions of switching between wired and wifi connections inside a NAT.

hmm.. well I think you can always use some sort of proxy, like VPN or an application proxy.
